Phalloplasty is usually a multi-stage journey. HYKR therefore uses stage-based timing, with each covered stage protected separately and a master pathway that may stay active longer when stages are pre-certified together.

EARLY RECOVERY

Healing is not automatically a complication

Swelling, bruising, sensitivity, and other expected recovery findings may fall inside the healing period rather than qualify as a care request.

Eligible issues must first manifest after the certificate date

The exact waiting-period end date appears on the patient certificate. An issue that first appears before that date does not become eligible simply because treatment happens later.

Corrective-care window remains active

Transfeminine protection may run through the selected 12–24 month tier. Phalloplasty may be protected 24 months per covered stage, with a longer master staged pathway when pre-certified.

Emergency ruleEmergency treatment should never be delayed while waiting for HYKR authorization. Emergency stabilization can still be subject to certificate terms, documentation, limits, and exclusions.

The number of allowed care requests is not an automatic entitlement. Every request remains subject to the certificate, waiting period, medical necessity, causation, documentation, approval, and remaining benefit limit.

What can trigger support

A care request is about a documented, medically justified issue—not dissatisfaction alone.

May be reviewable when the certificate requirements are met

  • The issue is directly attributable to the covered procedure or covered stage.
  • The issue first objectively manifests after the applicable waiting-period end date.
  • A corrective intervention is medically necessary and supported by documentation.
  • The procedure was performed by an eligible, approved provider.
  • HYKR reviews and pre-approves non-emergency diagnostics, corrective treatment, travel, or related expenses.
  • The request remains within the selected tier’s support limit and care-request allowance.

Examples of what HYKR does not support

  • Provider negligence, surgical malpractice, or improper technique.
  • Pre-existing conditions or known risks that existed before the procedure.
  • Subjective dissatisfaction, aesthetic preference, or non-medically necessary touch-ups.
  • Routine planned aftercare, standard follow-ups, or long-term care unrelated to a covered complication.
  • Failure to follow the treating clinician’s reasonable aftercare instructions.
  • Unauthorized treatment obtained before HYKR approval, except emergency stabilization.
  • Defective implants, devices, product costs, or manufacturer defects where another warranty/recall remedy is primary.
  • Costs above the remaining certificate limit.

What are the two HYKR gender-affirming surgery pathways?

The current program separates transfeminine SRS and FTM phalloplasty. Transfeminine examples include vaginoplasty, vulvoplasty, labiaplasty, clitoroplasty, and related approved procedures. The phalloplasty pathway can include flap reconstruction, urethral lengthening, scrotoplasty, glansplasty, and implant-stage work when specifically listed in the certificate.

How does protection work for a staged phalloplasty journey?

HYKR’s current program states 24 months per covered stage. When multiple stages are pre-certified under one plan, the master pathway may remain active for up to 48 months from Stage 1. The actual certificate controls the eligible stages, dates, limits, and waiting periods.

Does HYKR cover normal healing or routine aftercare?

No. Routine planned aftercare, standard follow-ups, and expected healing are not the same as a covered complication. The applicable waiting period also matters because the issue must first objectively manifest after the waiting-period end date.

What if I need urgent medical care?

Seek emergency care immediately. Do not delay stabilization while waiting for authorization. Emergency expenses are not automatically covered and remain subject to the certificate, documentation, selected tier, limits, and exclusions.

Does HYKR replace malpractice coverage or the surgeon’s responsibility?

No. HYKR does not cover provider negligence or malpractice and does not replace the provider’s clinical responsibility. The treating provider remains responsible for medical services and clinical judgment.

Is HYKR health insurance?

No. HYKR Assured is a warranty and patient-support program and does not constitute health insurance, medical insurance, or comprehensive healthcare coverage.
